Oftalmik ilaç ürünleri (ODP), gözün dış yüzeyine (topikal), göze içine (göz içi) veya bitişik (perioküler) uygulanmak üzere veya bir oftalmik cihazla birlikte kullanılmak üzere tasarlanmış özel dozaj formlarıdır. Yaygın oftalmik dozaj formları arasında sıvılar (göz damlaları, oftalmik solüsyonlar, süspansiyonlar), yarı katılar (jeller ve merhemler) ve katılar (ilaç maddeleri ile kaplanmış kontakt lensler, oküler ekler) bulunur.
Küresel düzenleyiciler, oftalmik solüsyonları ve süspansiyonları (OSS), ambalajlarından kaynaklanan olumsuz sonuçlar açısından yüksek riskli olarak görmektedir. Örneğin, FDA'nın 1999 Konteyner Kapatma Kılavuzunun Tablo 1'i, etkileşimden kaynaklanan bir paketleme-dozajının meydana gelme olasılığının yüksek olması ve uygulama yolu ile ilişkili yüksek derecede bir endişe temelinde, OSS'ye paketleme etkileşimleri için yüksek düzeyde bir endişe verir.
İlaç-ambalaj etkileşimi olasılığının daha düşük olması nedeniyle USP <1664>'te OSS için biraz daha düşük bir risk belirlenmiştir. Bu sınıflandırma göz önüne alındığında, FDA Rehberinin Tablo 2'si, OSS ambalajının hasta sağlığı üzerinde kabul edilebilir derecede küçük bir olumsuz etkiye sahip olduğunu belirlemek için USP Biyolojik Reaktivite Testinin ve muhtemelen ekstraksiyon/toksikolojik değerlendirmenin gerekli olduğunu belirtmektedir.

Oftalmik yarı katılar ve katı maddeler yönetmeliklerde özel olarak belirtilmemiş olsa da, ilaç-ambalaj etkileşimi olasılığının daha düşük olması nedeniyle muhtemelen daha düşük atanmış riske sahiptirler.
OSS ve ambalajlarıyla ilişkili yüksek riske ek olarak, OSS ve ambalajları, kimyasal ve biyolojik güvenlik değerlendirmelerini karmaşıklaştıran belirli benzersiz özelliklere sahiptir. Örneğin, OSS için en tipik paketleme formatı plastik (düşük ve yüksek yoğunluklu polietilen; LDPE, HDPE) şişelerdir. Birincil ambalaj olarak PE malzemelerin bir eksikliği, kimyasal göçe karşı nispeten zayıf bariyerler olmalarıdır. Bu nedenle, paketlenmiş OSS, şişe üzerindeki baskı veya etiketler, ikincil, üçüncül ve yardımcı ambalajlar (üst torbalar, kartonlar, şrink ambalaj) ve depolama/dağıtım ortamı ( örneğin, nakliye paletleri).
Bu zorluklara rağmen, oftalmik solüsyonlar ve süspansiyonlar, güvenli olmayan sızıntı riskini azaltan çeşitli avantajlara sahiptir: düşük günlük doz hacmi, farklı bir toksisite profili ve raporlama eşiklerine farklı bir yaklaşım. Birinci avantaj göz önüne alındığında, oftalmik solüsyonlar ve süspansiyonlar için günlük doz hacmi tipik olarak 1 mL'den azdır. Günlük doz hacmi bu kadar düşük olduğunda, OSS'deki "güvenli" bir sızabilir madde konsantrasyonu genellikle diğer, daha yüksek günlük doz hacimli ürünlerdeki (örneğin, büyük hacimli bir parenteral) bu tür bir konsantrasyondan daha yüksek olacaktır.
İkinci avantaj göz önüne alındığında, PQRI PODP tavsiyeleri, oküler ilaçların yüksek bir lokal maruziyet ve buna karşılık gelen düşük sistemik maruziyet elde etmek için tasarlandığını not eder. Bu nedenle, lokal topikal (oküler) etkiler, ODP'ler için toksisite potansiyeli için birincil bir endişe alanı olarak kabul edilir ve genel ve gelişimsel ve üreme toksisitesi dahil sistemik etkiler (diğer ürün türlerine kıyasla) ODP ile daha az alakalıdır. Bu nedenle, yerel etkiler için güvenlik eşikleri (yani, hassaslaşma ve tahriş/toksisite), ODP'lerin sızabilir ve ekstrakte edilebilirlerini nitelendirirken birincil değerlendirme olacaktır. Bununla birlikte, genotoksisite potansiyeli de bu ürün tipi için ilgili bir son nokta olarak kabul edilir.
Duyarlılık/tahriş için güvenlik eşiği, mutajenite ve kanserojenlik gibi sistemik etkiler için olandan genellikle daha yüksek olduğundan (örneğin, PODP tavsiyelerine göre günde 5 µg/gün'e karşı 1.5 µg/gün) bu durum önemlidir.
Son olarak, birçok ilaç ürünü dozaj formu için bir ÖTV türetilebilirken, bir ÖTV oluşturmanın temeli olacak olan ODP'lerle (yani duyarlılık, oküler tahriş) ilgili toksisite son noktalarının bir veri tabanı şu anda mevcut değildir. Bu nedenle parenteral ürünler için kabul edilen 1.5 µg/gün ÖTV doğrudan ODP'ye uygulanamaz.
ÖTV, ekstrakte edilebilir ve sızabilir taramada “ne kadar düşük gidiyorsunuz” sorusuna cevap verdiğinden, ilk bakışta bu durum bir dezavantaj gibi görünebilir. ÖTV'ye sahip olmamak, tek geçerli cevabın "yöntemlerinizin gidebildiği kadar alçaltın" olduğu anlamına gelir; bu, ODP'lerin çok düşük seviyelerde sızıntılara karşı taranması gerekeceği ve birçok tanımlanamayan sızıntının ortaya çıkabileceği olasılığını sunar. Bu sunumun içeriği kamuya açık olmasına rağmen, bu eşikler resmi FDA kılavuzlarında yer almamaktadır. Bu nedenle, PODP tavsiyelerinde, olası bir yaklaşım veya başlangıç noktası hakkında fikir vermek için bir referans olarak tanımlanmaktadırlar, ancak takip edilmesi gereken bir uygulama olarak değiller.
Bu tanımlama ve yeterlilik eşikleri genellikle örneğin ÖTV gibi parenteral eşikler kullanılarak hesaplanacak olandan daha yüksek olduğundan, bu ODP'ler için bir avantajdır.
